Heart Disease Prevention

Nolan Family Center for Cardiovascular Health

We’re focused on preventing heart disease before it happens and developing strategies that strengthen heart health for patients everywhere.

Creating a World Without Heart Disease

The Importance of Prevention

Heart disease is the leading cause of death for men and women in the U.S.—but it doesn’t have to be. Up to 80% of cases can be prevented through healthy lifestyle choices.

MHIF is advancing prevention through innovative research that uses data to better identify risk, understand early warning signs, and uncover the most effective ways to keep hearts healthy. With world-class expertise across all cardiovascular specialties, regional and global collaborations, and access to big data, we’re transforming the future of heart health—one prevention breakthrough at a time.

Committed to Groundbreaking Prevention Research

The Nolan Family Center for Cardiovascular Health, led by Dr. Michael Miedema, is driving innovation in cardiovascular prevention research. Building on MHIF’s long-standing legacy of discovery and education, the center explores a wide range of prevention topics—such as improving cardiovascular risk prediction with coronary artery calcium testing, evaluating blood pressure and cholesterol guidelines, optimizing statin therapy, and studying the role of nutrition and healthy lifestyle behaviors.

Thank You to the Nolan Family

We’re grateful to the Nolan family for the $5 million charitable gift to accelerate progress in world-class cardiovascular prevention research and education–creating the Nolan Family Center for Cardiovascular Health.

Cardiovascular Risk Prediction

Importance of Prevention Efforts

A key goal of cardiovascular prevention is to identify risk early—essentially creating a “crystal ball” for heart health. Understanding which patients need proactive care can help prevent heart attacks and avoid invasive procedures.

Coronary Plaque Assessment

Using advanced Computed Tomograph (CT) imaging and Artificial Intelligence (AI) technology, we can now visualize hard and soft plaque in the arteries. Our research explores whether this innovation—combined with a proprietary algorithm—can better predict and prevent future heart disease.

Test 2 Treat

This study ensures every patient hospitalized for a heart event has their LDL (“bad” cholesterol) checked before discharge, understands their target goal, and begins the right therapy. Some patients also connect with a virtual “care champion” for follow-up support and medication guidance.

Familial Hypercholesterolemia (FH) Patient Registry

Through the national CASCADE-FH Registry, we partner with researchers to improve awareness, treatment, and outcomes for people with inherited high cholesterol.

Optimal Preventative Therapies

Even with significant improvement in cardiovascular risk prediction, progress in Cardiovascular Disease (CVD) prevention cannot be made unless effective treatments are available. Our goal is to be involved in a broad range of studies evaluating treatments targeting multiple risk factors, including hypertension, diabetes, lipids, smoking, nutrition and obesity.

This annual lecture honors the work of Dr. Kevin J. Graham and features international leaders in the field of cardiovascular prevention.

A tireless and innovative thought leader in cardiology, Dr. Graham served as president of the Minneapolis Heart Institute and the Cardiovascular Services Division of Abbott Northwestern Hospital from 2007 to 2012. He established and led the Preventive Cardiology practice and was Director of Outpatient Clinical Laboratory at the Minneapolis Heart.

MHIF is leading clinical trials enrolling thousands of patients to study new cardiovascular medications. These include next-generation cholesterol treatments like PCSK9 inhibitors and promising diabetes drugs such as SGLT2 inhibitors and GLP-1 therapies—both shown to reduce heart disease risk.

Many patients struggle to take statins, the most common cholesterol-lowering drug. Our DESIFOR Trial tested ways to confirm true statin intolerance and found that two-thirds of patients previously unable to take statins could tolerate them after participating. We’re now expanding this study to more sites, aiming to enroll 100 patients and improve care for those facing this challenge.

The Heart of New Ulm Project (HONU) integrated and connected a wide variety of programs, strategies, services and evidence-based interventions simultaneously across all sectors of the community — health care, community and worksites. By tailoring policies, systems and changes to environments based on the community’s unique needs and culture, HONU became woven into the fabric of the community and achieved impressive health outcomes.

Unlike many research projects, as a result of the success of the partnership with MHIF during HONU’s research phase, New Ulm garnered the support of the community to keep investing in and sustaining the project for the future. The Heart of New Ulm continues today as a community-owned health initiative with a highly engaged leadership team and more than 80 community volunteers representing 35 organizations serving on various action teams.

Resources Specific for Women

Too often, women are worried about the health of their family or others around them. We translate our learnings into best practices for health care providers and women of all ages.

Hear From Our Patients

“Even in the first month of participating in the study, I learned how different cardiologists view high blood pressure and its causes and effects.”
