This consensus document provides expert guidance for hospitals and physicians managing patients with a severe type of heart attack who are being transferred for emergency stenting. It addresses timing, logistics, and decision-making to ensure patients receive timely and effective care. The recommendations aim to standardize practice and improve outcomes in these high-risk situations.

Traditionally, stenting the celiac artery in patients with median arcuate ligament compression has been avoided because of concerns about stent fracture and compromised blood flow to abdominal organs. However, during complex endovascular repair of thoracoabdominal aortic aneurysms, the celiac artery must often be stented to successfully treat the aneurysm. This study shows that this compression does not negatively affect long-term vessel patency, device durability, or survival, supporting the safety of complex endovascular repair in these patients. 

The long-term impact of mixing devices from different manufacturers to treat the same patient had not previously been published. This study shows that using endovascular stent graft components from different manufacturers (“mix-and-match”) can be a safe and effective option for treating complex aortic aneurysms in the mid-term. This approach can help physicians treat patients whose anatomy may not fit a single device system and can increase flexibility when certain devices are unavailable. The findings support the careful use of mixed devices to expand treatment options for patients with complex aortic aneurysms.
