A landmark analysis from the International Registry of Acute Aortic Dissection (IRAD) challenges long‑standing assumptions about early mortality in type A acute aortic dissection. Reviewing data from 5,611 patients, the study found that modern early‑era mortality within the first 48 hours is 5.8%, far lower than the historically cited 1–2% per hour. Medically managed patients remain at high risk, with 23.7% mortality at 48 hours, while those intended for surgery show significantly better outcomes, with 4.4% mortality in the same time frame. These updated insights highlight the critical importance of timely surgical intervention and provide the most contemporary understanding of early risk in this life‑threatening condition
